Ask A Therapist: How Do I Help My Suicidal Best Friend?

A few months ago me and my best friends ex boyfriend (who still cares a lot about her) went to our school guidance councilors and told them how my best friend was suicidal. they told her parents and she had to get an evaluation from a therapist. they cleared her and she was allowed back in school. however now school isn’t in session and she’s suicidal again. I know this because she told me that I’m the only thing keeping her alive. a few years ago she was raped by a close friend and then a few days after the rape walked in on him killing himself. she never dealt with this traumatic event and I think it’s one of the reasons she’s suicidal now. we talked a little about it and she told me she feels like she messes everything up and all she does is make things worse. I tried to show her how that’s not true and how a lot of people care about her but she doesn’t believe me. I don’t want to go to her parents again because I dont think they’d believe me a second time. I want her to get help and talk to someone but I don’t know how to do it. please help me.

A: Thank you for your email. I can feel your concern for your friend through this letter. Even though you might be putting your friendship at risk, I suggest you go talk to her parents. They need to know about the rape and that she walked in on the person who raped her committing suicide. Those are horrific traumas for a teenager to witness and she is in serious danger.  Please watch the video response for more tools to handle this painful situation.

Ask A Therapist: I Get Serious Anxiety Before Therapy

I do therapy over the phone, and my stomach gets really hard and tight before I call my therapist. I get a ton of other physical symptoms, including muscle tension, diarrhea, tightness in my chest, shallow breathing, nausea, etc. I’ve talked it over with him, as I think it might be a transference thing, but it’s getting to the point of not being okay. I’ve had nervousness with other men, particularly doctors and people in an authoritative position, but I’ve never had it this bad. The kind of therapy I do is based on primal theory as well as other things, but it also makes it very hard for me to feel my feelings when I get so nervous around him. I don’t really want to switch therapists right now, but I would appreciate any suggestions!

A: Thank you for writing in with your question. What you’re describing sounds like a panic attack. Please talk to your therapist about possibly making some shifts in treatment, like meeting face to face instead of talking by phone, or getting a medication evaluation. While therapy is often uncomfortable, my concern is that your anxiety may be so overwhelming that it is getting in the way of your progress. Watch the video below for my complete response.

Ask A Therapist: Is This Depression, Personality Disorder, or Bipolar?

Well I’m 19, but I don’t feel 19. I have so many things going on in my life that it’s hard to keep up with everything. I’m a full time worker, a full time student and a part time gym rat. I’m also in a relationship. There is no time in the day for me to do anything and everything I do always feels rushed. Even though I’m interacting with my coworkers, friends, or girlfriend during the day, I feel empty and numb to it all, like everything is just an act. As far as feelings go, like I said, I’m numb. I feel as if my best friend or mother could die and I wouldn’t care, and I feel as though to a certain extent that I don’t care even for my girlfriend. But on the flip side, I don’t want to be alone. It scares me to think that me and my girlfriend would breakup. I laugh and joke but don’t know why I do.
I really want to know what’s wrong with me because I was never like this before. Or if I was, it was deep down and is now just surfacing and I can’t handle it. I WANT TO BE HAPPY AGAIN.

A: There are normal ups and downs that come along with adolescence, such as mood swings and hormonal changes. But what you’re describing sounds like a lot more than that. Please watch the video below for my complete response.

Ask A Therapist: I’m So Lonely But I Should Be Happy

Simply put, I’m very lonely.
I should be happy.  I just graduated college and starting my masters’ degree.  I own my own home. I have a job. I’m very fit. I try to do things that are social, but most of the things I do are by myself because I don’t have friends.
I’ve always had a boyfriend, and finally realized that they were a crutch, so I’ve been single for the past 2 years and concentrated on myself and my education. But being alone night after night and not having anyone to do things with is terrible. I’m considerate and friendly, but I just don’t know how to find friends. Seems like I’ve even lost my mojo in dating. I don’t feel like the person I used to be, and maybe I’m not anymore in a good way, but I can’t figure out why I don’t have any friends and am living a solitary life. It’s so lonely. What can I do?

A: I have more questions than I have answers. How long have you struggled with friendships? Has this been a pattern even when you were younger in elementary and junior high or is this recent? No matter where you live you can find friends. Watch the video for some suggestions for overcoming this loneliness and sadness.

Ask A Therapist: I Hate Myself And I Don’t Know Why

I hate myself and I don’t know why. How do I learn to love myself? Even though I believe I’m a daughter of God, I feel like believing and knowing is different than feeling. I don’t FEEL like that. I have urges to cut myself and sometimes give in, and I make myself throw up off and on. I hate being like this. I was sexually abused by a family friend for about six years. Even though he stopped when I got older, I never said anything to anyone. I feel like this might contribute to my feelings of hatred toward myself. Sometimes, I even think that my life has no purpose and that the world would be better off without me. I hate myself for doing things like spending money on a nice haircut. Every time I treat myself nice, even if it’s something like a bubble bath or chewing a stick of gum, I feel guilty. I treat other people well. I give people more energy than I have and it’s not fair to them or me. I know that if I treat myself better, I’ll have more energy to not only give to myself, but to others too. However, every time I try to do this, I end up cutting or throwing up because the urge to do so is overwhelming. How do I learn to treat myself well? What is your advice? Is there something I can do without therapy? I don’t have a lot of money and am out of a job.

A: Thank you for writing in and trusting me with your story.  I want to suggest to you that you look into the mental health resources at your college and so you can start on a path to healing. Many schools offer free or reduced fee counseling for students. I want you to know that you are not alone. You are not crazy. You have suffered years of sexual abuse. I’ve worked with many young adults who’ve been sexually abused and who’ve expressed similar feelings of self-loathing, cutting, eating issues, and emptiness.  Watch the video for more suggestions on how to start healing from your trauma.

Ask A Therapist: I Think I’m Depressed

So five months ago, I moved from Chicago to a new school. I thought it would be great to move to a new school, but I was wrong. It isn’t and I’m still not making any close friends. Everyone already has all their groups and best friends, so it’s really hard for me. I still haven’t found anyone to fit in with. And now, I’m starting to have self esteem issues. I was already having suicidal thoughts a little before I came to this school, but then they kind of died off. But about a month after being in my new school, the thoughts came back and stayed with me for another good 3 months. Even though these thoughts are gone, my self-esteem isn’t as great as it used to be. I cry a lot more often just thinking about my life, and I’m a lot more sensitive than I used to be. I also used to have an eating disorder, where I made myself throw up a few days a week. I barely read anymore, which is something I used to do all the time.
From this, do you think I’m depressed?

A: The short answer is yes, your symptoms do sound like depression. I suggest that you seek help immediately. The social isolation and low self-esteem, frequent crying, and your history of suicidal thoughts are serious symptoms that need to be addressed. While I can’t diagnose you in this forum, I suggest that you get a thorough mental health evaluation to find out exactly what’s going on, and what treatment you need so you can start enjoying your life. Watch the video below for my complete response. Thanks for writing in.

Ask A Therapist: Will PTSD Symptoms Jeopardize Military Career?

Q: I’m 23 years old and in the military. Recently I was raped while on duty, I haven’t been handling it well it brought up a lot of childhood stuff. I started seeing a psychologist, but I’m having a really difficult time opening up. She’s nice and I like her, but I don’t want to tell her too much, hurt my career and depend on her to keep my confidences when she can’t. I don’t know how to tell her about the purging or even if I should, she’s asked about the cutting but I don’t know what to say. I’ve been diagnosed with PTSD but I don’t want to tell her that when I have a nightmare when I wake up I can still see and feel what was happening in the dream. How do you open up and not come off as crazy? Please help me I could really use the guidance.

Ask A Therapist: Could This Be OCD?

Q: OK, I been having a problem where if I see someone that’s uneven or something it will bother me greatly and my mind tells me that if I don’t fix it something bad will happen to my mom. It’s weird, but say I’m reading a book and I skip a word my kind tells me to go back and start over or something will happen to my mom. There’s many other things that I will “fix” but why would this happen towards my mom? And do I have OCD? Should I talk to my mom about it, do I have OCD? Thank you.

Ask A Therapist: How Do I Open up Emotionally to my Therapist?

How do I open up to my therapist? I am constantly worried that he might think I’m trying to get attention. I have an eating disorder and I’m slightly overweight according to my BMI. I’ve overexercised, took laxatives, and most recently diet pills. I ended up giving my therapist one session because I was taking so many that it made me fell terrible. I’m just not able to be truly open and honest. He really is a great therapist, and I have a deeper connection with him than most others in my life. I have these feelings outside of therapy, but when I go in, I put on a face that everything is ok. How do I work on this to communicate better?

A: Great question! The emotional pattern of guarding your feelings is likely part of the reason you’re in therapy in the first place. Watch the video below for complete answer.

Ask A Therapist: I’m Madly In Love with My Ex-Therapist

I am madly in love with my ex-therapist…this is not transference, I truly love her! I never had the chance to tell her and now we no longer talk to one another. This has and is still bothering me. I can’t stop thinking about her and it is killing me inside everyday! I wanted to tell her back then during sessions, but was afraid to and now I will never have the opportunity to ever tell her. This is not healthy…what should I do? (28 year old female)
