Ask A Therapist: Am I Developing A Panic Disorder?

I just recently graduated high school and I currently don’t have a job. I spend both day and night inside the house either babysitting, watching TV, or writing on my novel. I don’t really have anyone to talk to besides my family because all of my friends have left for college. I’m beginning to feel pretty alone. Starting last month I have been having indigestion, trouble swallowing, a little bit of nausea, and my thoughts race out of control. Do the symptoms that I feel have anything to do with anxiety or is my boredom causing my mind to create symptoms? The symptoms are starting to happen when I get into a car to the point where I flat out refuse to even get into one. I’m afraid that I have the beginnings of panic disorder. Is this all in my
head?

Ask A Therapist: Can Employers Find Out I’ve Been Hospitalized?

In 2011 I was sent to a Behavioral Center because I tried to kill myself and I was self-harming very badly. I was released after a week and everything was fine. But now I’m trying to get a job and I’ve applied to many, but every time they fill it with someone who applied after me. I know I’m qualified for the jobs, so I’ve been wondering if they could see that I had been sent to the health center if I hadn’t told them? Please and thank you for your time.

A: Thanks so much for writing in. Assuming you’re in the United States, I don’t believe that your employer would have any way of knowing you have been in a behavioral health center. Our privacy laws are very strict and you would have to give permission to the behavioral center to release those records to the employer. For additional thoughts watch my video response.

Ask A Therapist: I Hate Myself And I Don’t Know Why

I hate myself and I don’t know why. How do I learn to love myself? Even though I believe I’m a daughter of God, I feel like believing and knowing is different than feeling. I don’t FEEL like that. I have urges to cut myself and sometimes give in, and I make myself throw up off and on. I hate being like this. I was sexually abused by a family friend for about six years. Even though he stopped when I got older, I never said anything to anyone. I feel like this might contribute to my feelings of hatred toward myself. Sometimes, I even think that my life has no purpose and that the world would be better off without me. I hate myself for doing things like spending money on a nice haircut. Every time I treat myself nice, even if it’s something like a bubble bath or chewing a stick of gum, I feel guilty. I treat other people well. I give people more energy than I have and it’s not fair to them or me. I know that if I treat myself better, I’ll have more energy to not only give to myself, but to others too. However, every time I try to do this, I end up cutting or throwing up because the urge to do so is overwhelming. How do I learn to treat myself well? What is your advice? Is there something I can do without therapy? I don’t have a lot of money and am out of a job.

A: Thank you for writing in and trusting me with your story.  I want to suggest to you that you look into the mental health resources at your college and so you can start on a path to healing. Many schools offer free or reduced fee counseling for students. I want you to know that you are not alone. You are not crazy. You have suffered years of sexual abuse. I’ve worked with many young adults who’ve been sexually abused and who’ve expressed similar feelings of self-loathing, cutting, eating issues, and emptiness.  Watch the video for more suggestions on how to start healing from your trauma.

Ask A Therapist: Low Self-Esteem, Technology Addict, and Fear of Relationships

Q: I feel like nothing I do matters and nobody really understands who I am. Every time I reach out to someone they let me down. I guess they just don’t care. The last few years I’ve taken to locking myself away in my bedroom to read or watch movies; it gives me more enjoyment than people do but I’m always feeling guilty about it too. I’m 19 years old and I’ve only kissed 4 guys ever, and never anything more. I’m afraid and self-conscious and I feel like I don’t get the opportunity to meet boys that other girls do. I know its my fault but its so hard to change, and I don’t know if I really want to be in a relationship anyway; I don’t think I’d be good at it at all. I’m always fighting with my parents, especially my dad; he yells at me a lot. I used to be so afraid of him when I was younger; he has quite the temper and is always criticizing me. My mother constantly nags me to go out more, to find a job, to stop watching so much TV, to eat better, to do more chores, to act older, the list goes on. I often get excited about little things and become quite childish and energetic, but the smallest thing can also send me into a spiral of sadness, anger or  frustration for the rest of the day. Both reactions seem to annoy my family. My few friends probably find it annoying too; if I could stand being thought ill of I’d probably ask  them. I always think if I were prettier or smarter or talented at anything, life would be better. I don’t want to be different or behind; I just wish things were easier. What should I do?

A: Thanks so much for writing in for help. The fact that you are reaching out for advice in this forum means you have some hope that things could be different for you, that you can feel differently about yourself and your life.

What you’re describing sounds like depression: social isolation, insecurities, withdrawing from activities, negative thoughts, hopelessness. First, I want you to go to your physician and have a physical to rule out any physical illness. While you’re there please talk to your doctor about your hopelessness, isolation and fears. See if medication is an option for you. Your tendency to turn toward technology may be a way to numb your emotional pain.

Also, ask your doctor for a referral to a psychotherapist in your area to work on ways to improve your mood, gain self-confidence, and gain relationship skills. You may also want to consider asking your parents to attend family therapy  to improve your family relationships.  Even though you’re 19, it sounds as if  you’re still stuck in experiencing the “childhood” disapproval of your father, and criticism of your mother, and letting those emotions dictate, on some level, how you feel about yourself. The good news is, you can feel differently.

Your family relationships greatly impact how you feel about other relationships.  If you think about your relationship with your dad as the “template” for male relationships, and you experienced him as scary and critical, then it makes sense that you would be hesitant to open up to other male relationships, like friendships and dating relationships.  It makes sense that you’d have only a few female friends, too, because you’ve experienced your mother as nagging and constantly correcting you. She is your model of how to relate to women so you likely may fear disapproval in your female friendships as well. Your therapist can help free you from these patterns so you can experience relationships with others differently, and not as extensions of your parental relationships.

In addition to meeting with your physician and therapist, I’d like to recommend a couple of books to you: “Feeling Good: The New Mood Therapy” by Dr. David Burns and “The Relationship Cure” by Dr. John Gottman. Both books will provide excellent tools and new perspectives on yourself and your relationships.

Ask A Therapist: I’m Sexually Attracted To My Therapist

Okay, so recently I started getting therapy for a few issues in my life and I have this therapist. Obviously, it’s their job to listen and what not, but it’s such a new thing for me to have someone listen and understand!! I have normal relationships and what not, but I don’t talk about what’s going on in my life. And well, like I said, this therapist, he listens!! And I don’t know how it started, but now I’m attracted to him and think about him all the time. Yikes! And I had therapy the other day and I was feeling a bit awkward sometimes because in my head I was thinking about him sexually!! And he was right there in front of me. Not good but so good at the same time! But yeah nothing will happen anyway because 1. he’s my therapist and that goes against code and 2. he’s married anyway. But it doesn’t stop me from thinking about him that way. I don’t know if I have control over my actions but don’t want to lose him as a therapist! And if I ask him for help about it, I guess I probably will. I can’t ask anyway… too awkward. And I don’t want to start again with a new therapist. So please give me some ideas! Oh, and if this helps in anyway, I have bipolar…. but I guess I’m not the first person in the world to be attracted to my therapist so maybe it doesn’t! (18 year old female who recently started therapy)

A: How wonderful that you are able to open up to your therapist and feel listened to. You are not alone in having a sexual attraction to your therapist and there’s a name for it — erotic transference — and it’s actually a quite common experience in therapy.  Transference can be worked through in the therapeutic relationship and that process can help you experience and resolve the deeper issues in your life. I hear that you are afraid of losing this relationship if you disclose your feelings to your therapist and that you’ll have to start all over.  Assuming your therapist is skilled and experienced, he will be able to help you work through this attraction.  Start slow in sharing your feelings. Let him know that you’re scared to share these feelings and that you want help to understand what they really mean.  When sharing your transference with your therapist, remember that your feelings are information about your deeper emotional longings, wounds, and needs, and are not about your actual relationship with your therapist. You’ll get through this. Thanks again for having the courage to write to “Ask the Therapist.”

Ask A Therapist: I’m Caring for Mentally Ill Adult Brother

Q: If something was to happen to me my brother would not be able to survive.  I need help. My mother adopted my brother at 13 whom is 21 now.  He had been foster care since he was 2 years old.  They labeled him as mentally ill.  We would hit is head on the wall, get upset and not talk for hours, and walk with his head shaking and hand dangling.  He was in LD classes in school and had visited 33 different schools in his lifetime.  Growing up he moved to main stream classes and currently he is in his 3rd year in college but just started taking regular classes.  My mother passed away in 2007 and it was left to my dad to raise him.  My dad tried to get him help and was told that he could take care of himself.  My dad could not handle it any longer so I took him in.  It took him 7 times to pass his test to get his license.  He does maintain a dish washing job.  The best I can discribe him is he can do things but needs to be reminded and has no sense of reasoning.  Only follows directions but will follow them exactly.  Just yesterday he didn’t understand that if he couldn’t make it to work that he had to let them know.  He thought he could just go in the next day and tell them.  I have remind him to clean his room, and he isn’t apart of the household.  He just stays in his room all the time.  I have realized he needs someone for a lifetime and I can’t provide it.  I am a single mother of three girls and need help.  He needs help with his finances.  I didn’t realize that til he was 500 dollars in the hole and wasn’t paying any bills.  I just don’t know where to start to get him the help that he needs.  Please help.

A: Thanks for writing in for help. I want to commend your for taking in your brother. That is very courageous. It sounds extremely difficult to see no end in sight, and to know how vulnerable he would be in the world without someone to guide and support him. I believe that there are two things that need to happen at this point: 1) access additional support for your brother and 2) find  help and relief for you so you don’t completely burn out.

Does your brother have an official diagnosis? If not, I recommend that you take your brother in for psychological testing and evaluation. Depending on his diagnosis, he may be eligible for additional resources and care through your state, and may qualify for disability benefits. Please consider contacting NAMI (National Alliance for Mental Illness) in North Carolina and inquire about advocacy and support services in your area, and contact your community social services agency here. Your brother may be eligible for some type of supervised housing situation, occupational therapy for life management skills, and other services. Putting some long-term help in place for your brother will hopefully alleviate some of your current burden and decrease your concerns about him if something should happen to you.

In the short run let’s get you some additional help. Can your father take shifts caring for your brother to give you a break on a regular basis? Are there any adult day care services in your area where you could know he was safe? There are resources available. Please reach out for support for both of you.
